UnityPoint Health® – Deer Creek Family Care Clinic in Toledo is set to merge with UnityPoint Health Family Medicine – Tama/Toledo. The two clinics will operate together as UnityPoint Health Family Medicine – Tama/Toledo from the clinic location at 1307 S. Broadway, Toledo beginning July 8.

Deer Creek Family Care and Family Medicine – Tama/Toledo are both part of the larger UnityPoint Health family and share the same mission – to provide the best outcome for every patient, every time. Starting July 8, current physicians and team members of both clinics will provide care from one shared location.
“The merger of our clinics will allow us to better serve our patients,” says Shari King, vice president of operations at UnityPoint Health – Marshalltown. “Our physicians and clinical staff are skilled in providing exceptional primary care and will continue to do so as a larger clinic team. Existing patients will still get the same great care—nothing is changing except welcoming the Deer Creek Family Care team and patients to our shared location.”
