Health Information Management maintains your medical records and works to ensure that they are accurate, complete and confidential, as well as available for continuing care to those with a need to know. If you need copies of your records for yourself, or sent to other health care providers, we'd be happy to help. The department is open for non-emergent release of information requests Monday through Friday 8:00 a.m. to 4:30 p.m.

If you need a copy of your medical record, please visit MyUnityPoint, our secure patient website, to access your health information.

  • Information about registering for MyUnityPoint can be found by visiting chart.myunitypoint.org.
  • If you need assistance using or enrolling for MyUnityPoint, please call the support team at (877) 224-4430. 

You may also request a copy of your medical record by:

  • Contacting the Health Information Management Department at (641) 754-5044.

UnityPoint Health - Marshalltown Authorization for Medical Records:

If you would like to complete the following authorization and mail it to us, we can process your request for information.

For those of you with Advance Directives concerning end of life issues, we welcome any advance directive information that you may have prepared.
